Then there's the fact that on PC people could just give themselves shit through commands, join a co-op game and then ruin their world (either by giving them overpowered shit right away and destroying the difficulty or by killing NPCs). You could always not use VATS(I usually don't), but it's something that has to be considered. Then there's the whole thing of 'How would it work?' With VATS stopping time while you pick a target I can imagine how obnoxious it would be for 2 people trying to kill monsters constantly being frozen while the other guy shoots. I'm not against co-op, but I'd rather have a bug-free singleplayer game than a buggy singleplayer and co-op one.
